Visits by
appointment only. Cancellations must be made 24 hours in advance,
otherwise, the full fee will be charged for the reserved slot.
Payment and
Billing:
Fees are due at time of service. Level of service will determine the
cost of the visit. We accept cash, check, Visa, or Mastercard. Our
billing service is happy to process your insurance information for you.
Check with your insurance policy to see if Dr. Eisen is within your
company’s network.
Medicare:
We are a “Non-participating” Physician, but we ask that you pay the
portion of the charge that is the patient’s responsibility.
